The Clowes Fund Self-Evaluation Form
Use the Grantee Self-Evaluation Report form to submit the following:
- Interim Reports - due for multi-year grants to trigger annual payment. (Submit via e-mail to email@example.com.)
- Final Reports - due within 30 days of project completion, or one year following receipt of grant. (Submit via e-mail to firstname.lastname@example.org.)
- Status Reports - due with a new proposal unless a final or interim report has been submitted within six months of the proposal deadline (i.e. since Aug. 1, 2012) for any prior Clowes Fund grant. (Submit as an attachment to your application on eGrant.)
- Please note: You must submit a Grantee Self-Evaluation report on any previous grants from the Fund before a new proposal will be considered. If the previous grant period is not complete by the proposal deadline, you may file a status report. Organizations that have filed a status report within six months of the proposal deadline have fulfilled this requirement. A final report must be submitted before any payment for a new grant will be paid.
